The mission of the Bentonville West band program is to enrich the lives of students through musical excellence and performance while providing lifelong lessons.

Physical Examination REQUIRED

Deadline: Band July 22nd, Guard July 15

New this year: Per AAA rules, all students will be required to submit a Physical Examination.

AAA Physical Form - Must be turned in with Physician Signature

AAA Sports Physical History Form - For your doctor only (if needed)

All students in the Bentonville West Band Program are REQUIRED by the Arkansas Athletics Association (AAA) to have a physical completed. You can turn the form into the band office. You may submit a copy if one was already completed for athletics for the 2024-2025 school year.
